New Patients

New Patients
Patients are only considered “New” the very first time they visit our practice. All New Patients are required to complete the Patient Registration Form and Patient Health History. These forms are available here or can be sent via email or by post at your request. It is important that the information you provide on these forms be complete and accurate.

New Patient Appointments - Family Practice
This category applies to most new patients. Patients are asked to schedule five visits, typically spaced one week apart. For most patients these first five visits are the beginning of their healing process. They give Keith Bell, L.Ac. a clear picture of your health and how your body responds to treatment. The first visit is 1 ½ hours, during which Keith will spend time getting to know you, your medical history and forming a diagnosis in Traditional Chinese Medicine. The four follow-up visits are approximately 1 hour and allow you and Keith to discuss how you are responding to treatments. During each visit you will receive an Acupuncture treatment that generally last 30 to 40 minutes. During the 5th visit you and Keith will review your progress and discuss the best treatment plan for you as well as herbal formulas or lifestyle changes that may be useful in your healing process.

New Patient Appointments - Pediatrics /Minors
OMS offers treatment to children and young people for a variety of health problems. For patients under the age of 18, initial appointments must be scheduled by a legal guardian, who must also be present at the initial consultation and treatment. For children 12 and under the initial consultation is one hour and follow-up visits are typically scheduled for ½ hour (See Fee Schedule for cost). For children between the ages of 12 and 17, the length of the appointments will be determined by Keith Bell, L.Ac. on a case by case basis. If the child is over 16 years of age, they be permitted to schedule and receive follow-up treatments without a parent present provided both the acupuncturist and parent(s) approve and OMS has a valid credit card on file with written authorization from the parent/guardian for all charges.

New Patient Appointments – Fertility: Conscious Conceptions
Conscious Conceptions offers Holistic Preconception Planning and Pregnancy Support. All new Patients seeking treatment for issues related to Fertility should schedule a one hour Fertility Consult. Both the patient and their partner are encouraged to attend this consultation. Following this consult Keith Bell, L.Ac. will recommend a treatment plan and schedule based on the patient’s individual needs. However, new fertility patients are asked to make a commitment to treatment. Pre-Conception Planning and/or the treatment for infertility, typically begins with hormone and menstrual cycle regulation, a process taking at least three months.

New Patient Appointments - Sports Medicine (Acute Injuries Only)
New patients seeking treatment for only an acute injury will be scheduled for a one hour initial visit at the earliest possible time. If necessary the Acupuncturist will recommend a follow-up treatment schedule at the end of the first visit. New patients with chronic injuries and/or pain should follow the policy for New Patients: General/Family Practice.

Scheduling Appointments
How we schedule appointments is extremely important. Our system for scheduling, rescheduling and/or canceling appointments has been thoughtfully planned and time tested. It helps us manage a busy practice and stay focused on what is most important, you, the patient.

We require that all appointments be scheduled either in person or by phone. Over the years we have found this to be best way to avoid mistakes and miscommunication. For that reason, we do not schedule, reschedule or cancel appointments by mail, email, text message or any other means of communication. Also, appointments must be scheduled by the patient or by a legal guardian.
